Short-Term, Non-Academic Hourly Employee
If you have recently been offered a non-academic, part-time, or temporary assignment, and have only been previously employed as a Student Worker, or have not worked for the District in over one year, you will need to obtain clearance to work.
Human Resources will e-mail new hire forms to you via AdobeSign upon receiving a Human Resources Transaction Form (HRT) from the hiring department. Please note that along with the AdobeSign packet, the forms below will be e-mailed with further instructions as well. ALL the required documentation that is emailed to you must be completed and returned to HR prior to your first date of employment.
- TB Risk Assessment Form - Per and , District employees must be assessed every four years for Tuberculosis risk. A Tuberculosis Risk Assessment Form must be completed by a medical provider licensed in the state of California and submitted to HR.
- Live Scan Form - requires all candidates for employment to have fingerprints taken via the Live Scan fingerprinting service before receiving clearance to begin work. Please print and take the State of California Request for Live Scan form to a location offering Live Scan services, and return a copy of the completed form to HR. If you would like to be reimbursed for the cost of the Live Scan, please retain your receipt and submit it to HR . Typical cost for the Live Scan process ranges from $70-$90.
